Forms and Office Information

  • Interventional Pain Consultants is open Mon-Thurs 8am to 5pm, 8-12 on Fri.
  • A new patient visit is 1.5 hours for health history and treatment.
  • An established patient visit is 1 hour.
  • Appointments are booked on the hour or half hour throughout the day.
  • We accept Visa, Mastercard, Discover, personal checks, and cash
  • As a courtesty to our clients insurance plans are billed as well as auto injury claims
  • 24 hour cancellation notice is requested.
  • You can call for a same day appointment and we will make every effort to see you
If you are a new patient download and print the following forms. Fill your forms out and bring them with you to your appointment. If you have any medical records you feel are pertinent to your problem please bring them with you. If you are unable to fill out your forms before your visit please arrive at least 20 minutes prior to your appointment so that you complete them before your appointment.

You can expect that your appointments will occur on time. I make every effort to respect your schedule.
You will need Adobe Acrobat reader to view and print the forms. If you do not have Acrobat it is available for free at Adobe.com.

Payment for services is requested at the time of service. Interventional Pain Consultants will bill your insurance company if you have coverage for our services. Please check with your insurance plan to verify that you do. To contact your plan see your insurance ID card. On the backside of the card there will be a phone number to call and inquire about coverage.
